TERMS AND CONDITIONS FOR AWARDING THE „HEART OF THE EARTH” MEDAL
- GENERAL ARRANGEMENTS
- These terms and conditions were elaborated according to statute of „World Organisation of Koluchstyl” Foundation.
- The „Heart of the Earth” medal, hereinafter referred to as the „medal” is given by „World Organisation of Koluchstyl” Foundation.
- Medal approved by the „World Organisation of Koluchstyl” Foundation consists of the following parts:
- the face – diameter of the medal is 65mm. On a circular shape medal is a blue earth’s map. There is a golden heart in the middle of it. That golden heart is a heart of every man that brings help and hope to other people. There are two ribbons coming out from the bottom of the heart. They are white and red – Polish national
- reverse – golden with a title saying: „The Heart of the Earth medal” and a catalogue number below, enclosed also in certificate and engraved on additional metal plaque.
- medal is presented with a navy blue necklace ribbon attached.
- Graphical scheme of the medal is included (Attachment 1).
- RULES FOR AWARDING THE MEDAL
- This Medal is given to people and institutions from around the world who help and work for development of human and earth. Those people also work for human integration, which includes medical, humanitarian, sports and educational aid. They also strongly care about ecologic state of our planet and help people in many other ways.
- People and institutions nominated for the award should:
- stand out for their longstanding commitment in charity,
- sacrifice their free time and lives for others.
- Maximum of 7 medals are given out every 2 years.
- The medal can be received only once in a lifetime.
- Medal is given out together with a certificate and metal plaque.
- The person or institution awarded with the medal is recorded in the International Registry of the Medal and gets media coverage.
III. MEDAL GRANTING PROCEDURE
- International Council consists of 15 to 25 members from around the world.
- Members of Council are constituted by „World Organisation of Koluchstyl” Foundation indefinitely.
- „World Organisation of Koluchstyl” Foundation can withdraw any member of the International Council at any given time.
- The Chairman of the International Council is constituted by „World Organisation of Koluchstyl” Foundation.
- The International Council will elect a secretary during the first meeting in a simple majority voting.
- International Council members must be adult, non convicted, have good professional reputation, high morality and experience in teamwork.
- People and institutions can submit candidates both from their countries as well as candidates from abroad . It is also one of the tasks of members of the International Council.
- Application for medal granting should include:
- personal data,
- address,
- detailed information about candidate,
- outstanding achievements that qualify the candidate for receiving medal
- Application form is included (Attachment 2).
- Candidature must be submitted at least 3 months before gala. This rule does not apply to the first gala.
- Candidates must accept their nomination and sign approval for personal data processing. (Included in Attachment 3).
- The Medal is given out by the International Council by an act. Simple majority voting is only valid when at least half of members take part in it.
- International Council can make new acts during meetings in-person as well as via communication devices or written letters or e-mails. An act is valid only when all members of the International Council were informed about its content.
- MEDAL GIVING
- Person awarded with the medal will receive it together with a certificate.
- Medal giving will take place during gala that takes place every 2 years.
- The data of next gala is decided by the International Council by the act in simple majority voting.
- FINAL PROVISIONS
- In case of loss or damage of medal or certificate, Chairman of International Council may replace it on request of the awarded. The person requesting is obliged to pay all costs of making a duplicate.
- The Chairman of International Council may revoke award if it turns out that it was given due to a fraud.
- The secretary of International Council is in charge of International Registry of the Medal.
- Technical and organisational support is provided by „World Organisation of Koluchstyl” Foundation.
- These terms and conditions come into effect on the day of its constitution.

FOR AWARDING THE „HEART OF THE EARTH” MEDAL
Form of agreement to participate in the process of examination of the applications as well as possible granting the „Heart of the Earth” medal. Approval form for personal data processing during the process of examination of the applications as well as after receiving a medal.
Personal data provided by the person submitting the nomination for receiving the „Heart of the Earth” medal awarded by the International Council appointed by the „World Organisation of Koluchstyl” will be processed in order to consider the proposed candidature and possible granting the „Heart of the Earth” medal. The administrator of personal data will be the „World Organisation of Koluchstyl” (DANE ADRESOWE). „Personal data” includes: first name, last name , nationality , address , family status , professional and charity activity. In the case of granting a medal, personal information described above will be processed in order to promote the „Heart of the Earth” medal, charity and the „World Organisation of Koluchstyl” Foundation. Personal data will also be processed if necessary for legitimate purposes of the administrator of personal data. Each person providing personal data has a right to access their personal data and is allowed to correct , update, add and delete it . Data submission is voluntary. However, not providing data will result in inability to take part in granting of the „Heart of the Earth” medal.
Given the above information , I agree to my candidacy and possible granting of the „Heart of the Earth” medal as well as processing of my personal data by the „World Organisation of Koluchstyl” (in accordance with the Personnel Protection Act of 2002 No. 101, position 926) . I was informed about the right to access my personal data and I am allowed to correct , update , add and delete it.
